Elements of a Referral:
• Primary complaint - reason behind why the person is being referred to a
specialist
• A brief background of the problem - including symptoms observed over
time, how often these symptoms are seen, how intense these symptoms are
when seen
• Changes that happened since the problem began - how much has changed
from the person’s baseline level of functioning (changes that are seen in the
day to day activities, work or school performance, and interaction with other
people
• Things done to manage the present problem - home remedies, consult with
a spiritual advisor, medical practitioner, mental health practitioner, a traditional
faith healer or albularyo, laboratory tests, or medications taken
• Outcome of those interventions - was there an improvement or did the
condition worsen
